Log into the site by clicking on the tiny "Members Only" tab at the top. Enter your username and password, and press the Login button.

In the table in the bottom middle of the resulting page, you'll find "Direct link to the MOFC online entry for fish submission". Click the "Select here" link just to the right. Scroll down, and voila, you're there.
